Completing Your Paperwork
We want to respect your time from the moment you walk through our door. To help your appointment start on time, we encourage new patients to complete paperwork before arriving. When you schedule your appointment, ask our team about completing forms in advance.
If you'd prefer to fill everything out at the office, that's perfectly fine too — just plan to arrive 15 minutes before your scheduled appointment time so we can get everything processed without rushing your visit.

Meeting Dr. Smith or Dr. Zaman
Following your hygiene appointment, you will meet with Dr. Vernon Smith, DMD or Dr. Mohammad Zaman, DDS. Your doctor will:
- Review the results of your examination and X-rays with you
- Explain any findings in plain, straightforward language — no confusing dental jargon
- Walk you through any recommended treatments and clearly explain why each one is being suggested
